- RTO Restrictions: RTO must be included at time of submission
- Guaranteed Hours: facility is able to call off once per two week pay period
Job Details
- Certs REQ: ACLS, AWHONN Intermediate, BCLS, NRP, FHM
- Minimum of 2 references covering 1 year of employment total within the last three years within a comparable specialty/setting that they are booked
- Application must include at least the last 5 years of employment
- Physical Exam within 1 year of start
- Fit Mask within 1 year of start
- Tetanus/Diphtheria/Tdap within 10 years
- IGRA/TSPOT, QGold, or 2 PPDs within 12 months of start
- 2 MMR Vaccines or Positive Titer
- Hepatitis B documentation
- Varicella documentation

- SUBS: Aya I9 Attestation Required
- Government-Issued Photo ID
- Required for WA State OBRA search. OBRA search is required for CNAs at Skilled Nursing facilities (SNF) in WA ONLY.
- Aya E-Verify Attestation Required
- Must be AHA (Instructor, Provider, or RQI) or “Heart&Stroke” certification from Canadian Heart & Stroke Foundation ONLY. Must check UD to verify certifications required. If no UD is linked, defer to Clinical Tracks. CL - If UD or Clinical Track is not available at the time of qualification, please add dash note 'CL please audit requirements'.
