DRIVER

POSITION PURPOSE : To transport adolescents and adults (ages 13 - 99) with direct supervision and to always obey all traffic laws. Assist in the direct care and management of child, adolescents and adults (ages 4 to 99) and to provide supervision, safety and crisis management for client population.

KEY RESPONSIBILITIES :

1. All drivers will be certified and completed the Transportation Orientation and training.
2. Maintain an amount of order that allows for safe operation of the vehicle without distractions.
3. Insure that everyone in the vehicle is wearing a seatbelt whenever the vehicle is in operation.
4. Do not operate the vehicle unless everyone is belted in.
5. Pay attention to the road and any unusual conditions.
6. Operate the vehicle in a way to insure the highest degree of safety.
7. Obey all traffic laws.
8. Patient safety and direct supervision is the number one priority.
9. Report all behavioral problems and sexual issues to Transportation Coordinator.
10. Report any problems or concerns on vehicles.
11. Take steps to prevent accidents.
12. Be on time to work and all physician appointments.
13. At all times, staff shall respect the rights of the patient.
14. Will follow dress code of Healthcare America and Transportation Department.
15. Laurel Ridge cell phone is on at all times.
16. When picking up or dropping off patients, the first patient that you pick up, as well as the

last patient that you drop off is the same gender as the driver.

1. Under direct supervision assists in the development and implementation of the treatment

plan for each client.

1. Responds to crisis situations.
2. Assists in providing safe delivery of care to promote wellness.
3. Assists with unit milieu.
4. Ensure documentation of client behaviors.
5. Acts as safety officer on unit and submits reports in accordance with policy and procedures.
6. Adheres to facility philosophy and general policies.
7. Provides transportation to clients in accordance with hospital policies and procedures.
8. Ensures that all clients are provided with an appropriate meal according to physician diet orders.
9. Perform other duties as assigned.

QUALIFICATIONS :

SKILLS, KNOWLEDGE AND ABILITIES: Current Texas driver's license; good verbal and written skills; high school graduate or equivalent; must be 18 years of age; ability to drive a 15 passenger van and car. CPR certification. Knowledge of the City.

EDUCATION: High school graduate or equivalent.

LICENSURE/CERTIFICATION: Current Texas driver's license.

EXPERIENCE: One year of experience as a Mental Health Worker preferred with adolescents (ages 12-18) or child or adult.

PHYSICAL REQUIREMENTS:

* Must be able to lift and carry up to 50 pounds.

* 80% - 90% of the time, the incumbent is standing, walking, kneeling, and reaching while working with the patient/residents.

QUALIFICATIONS : Continued

* Requires full range of body motion including handling and physical restraint of patient/residents.

* Ability to perform CPR.

* Ability to work split shifts.

* Emotional stability necessary to perform essential functions.

* Vision and hearing sufficient to perform essential functions.

* Ability to drive a vehicle.

* Ability to speak and write.
