POSITION SUMMARY:
Under the direction of the ADS Program Director this position drives a minibus, van, or lightweight vehicle to transport Adult Day Services participants or physically limited passengers and deliver them to their destination. A driver may also be expected to transport agency personnel. The driver is expected to keep accurate vehicle records, maintain the vehicles in a clean, safe, and orderly condition.

Chicago Commons provides services that help seniors and adults with disabilities maintain their independence and quality of life while remaining in their homes. Our Adult Day Service (ADS) program provides a professional care setting in which seniors and adults with disabilities are transported to our center to receive individualized care in a community-based group setting. Our Adult Day Service center is located in Bronzeville and provides daily onsite care for seniors and adults with disabilities.
ESSENTIAL DUTIES & RESPONSIBILITIES:
These include but are not limited to the following.(The company reserves the right to add to, delete, change or modify the essential duties and responsibilities at any time).
Arrange for vehicle repair with the assistance of the ADS Manager. Follow established schedules; adjusts schedules to meet unanticipated changes; determines the safest and most efficient pick-up and delivery routes and other destinations according to schedule.
Responsibilities:
•Assist passengers on and off the vehicle safely and courteously. Assist clients who use medical equipment, and handle emergencies that may arise.
•Provide support, and assistance and always interact with participants during field trips and activities in addition to pick up and drop off.
•Provide support in the activity rooms if needed.
•Ensure the van is clean and in optimal condition for the comfort of the clients.
•Follow the daily schedule assigned to transport clients to numerous areas within the city of Chicago.
•Provide a high level of customer service to clients enrolled in the program.
•Note visible changes in the physical or mental health of clients regularly transported; communicate this information to appropriate personnel for follow-up.Document unusual behavior.
•Secures wheelchairs with restraining devices to stabilize wheelchairs during transport.
•Ensure that clients are received by the caregiver and/or authorized person.
•Complete daily routes report.
•Report and Complete Incident report in the event of any vehicle accidents.
•Conduct weekly vehicle inspections. Thoroughly inspect the vehicle and maintain a clean and safe condition.
•Report the need for maintenance and/or service to the ADS Manager.
•Maintain standard records such as logs of passengers, mileage, and gasoline purchases.
•Operates radio or similar device to communicate with the base station, other drivers, caregivers or to report disruption of service.
•Follow at all times the ADS Driver manual.
•Always ensure safety of clients
•Must operate the van in accordance with vehicle and transportation laws, ordinances, and regulations.
•Other job duties are assigned.
QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.The requirements listed below are representative of the knowledge, skill, and/or ability required.
Valid Illinois Drivers License.
5-years of experience working with seniors or adults with disabilities.
5-years years of driving experience.
EDUCATION:
High School diploma/GED.
CPR certification is required.
WORKING CONDITIONS:
There are no unusual environmental conditions other than inclement weather and inappropriate driving of other vehicles.

- The employee must be able to read English proficiently.
- Must be able to assist with the transfer of ADS participants. This could require lifting.
- The employee frequently is required to sit and use hands and fingers to handle, or feel objects, tools, or controls. The employee is occasionally required to stand; walk; reach with hands and arms, and stoop, kneel, crouch, or crawl.
- The employee must occasionally lift and/or move up to 50 pounds. Specific vision requirements include the ability to see at close range.
