Job Description Summary:Job Description:The MHU Driver/CHW is responsible for the safe and efficient driving, maintenance and safety of the MHU and the support of clinical and program staff at each site. The MHU Driver/CHW will work directly with the Program Coordinator to support daily operations of the MHU program including driving, maintenance, stocking, clerical/administrative duties, scheduling, enrollment, outreach, and patient education.ROLE AND RESPONSIBILITIESDrive the MHU from assigned parking/storage at CHCI to scheduled locations across the state.Complete MHU Inspection reports before and after each deployment and report any concerns or challenges as appropriate.Work with the MHU Program Coordinator to develop policies, procedures, manuals, and training as needed.Assist the MHU Program Coordinator with all aspects of compliance for all safety and regulatory requirements for DPH license.Complete and monitor data entry, record keeping, and reporting.Conduct and maintain community outreach and collaboration with community organizations and partnerships.Develop and implement schedule for MHU locations and services.Assist with client enrollment and participation.Act as a patient advocate for individuals experiencing challenges that include social determinants of health like housing insecurity, food insecurity, and economic vulnerability.Coordinate equipment transports with CHC Facilities department as needed.Actively participate in all meetings related to MHU Program and CKP.Maintain comprehensive records and log books for MHU Program.Monitor MHU 1&2 vehicle maintenance, safety updates, cleaning, sanitation and all other physical attributes of vehicles.Monitor inventory of supplies and equipment for MHU 1&2.Performs other related duties as assignedQUALITFICATIONS AND PREFERRED SKILLSAssociates Degree in Human Services or related field or high school diploma/GED and adequate experience to replace this.Valid Connecticut Driver’s License and Clean driving record for at least 5 years.Prior experience working with community agencies and programs.Demonstrates ability to work cooperatively with providers and agencies.Effective oral and written communication skills.Prior experience in providing services to bicultural individuals/families desired.Ability to organize, prioritize, and maintain deadlinesWorking knowledge of the program, its target populations and additional resources available in the community.CHC requires as a condition of employment current American Red Cross CPR for the Professional Rescuer and AED (CPR/FPR/AED) certification. The only acceptable alternative is current American Heart Association BLS/AED for Healthcare Providers certificationPHYSICAL REQUIREMENTS/WORK ENVIRONMENTThis position requires some physical exertion, mostly in support of groups in a program area.Work is both remote, office-based and community-basedWORK SCHEDULE DEMANDSFull-Time, 40 hours a week with evenings and/or weekends required based on program needs. Ability to travel to locations as deemed necessaryCOMMUNICATION SKILLSExperience with Microsoft Excel, Word, and Outlook. This position is highly involved with staff, providers, clients, colleagues, and community. Strong oral and written skills are required.CONFIDENTIALITY OF INFORMATIONConfidentiality of patient and business information is a requirement.
