• DMV Printout (K4 DRIVER HISTORY RECORD FROM DEPT. OF MOTOR VEHICLES )
• Driver's License Copy (CLASS B )
• Proof of HS Graduation (HIGH SCHOOL DIPLOMA OR EQUIVALENCY )
